Upon admission, each new resident receives a comprehensive assessment to determine the best plan of care. Our multidisciplinary care plan team includes a physician, licensed nurses, therapists, social workers and dietitians. Along with our residents and their families, we establish goals for achieving and maintaining optimal independence. Every resident’s progress is reviewed regularly.
